Ethics Application Form

Please complete the University's Ethics Application Form for all applications to the University Ethics Committee (UEC), Department Ethics Committees (DEC) and School Ethics Committees (SEC). Please refer to the guidance document below when completing the application form, it has helpful information for each question.

The Code of Practice on Investigations Involving Human Beings (pdf) specifies which applications must be sent to the UEC for approval. If your application is for the University Ethics Committee, please send your the completed application form (with electronic signature) to ethics@strath.ac.uk in the first instance. Otherwise, please send your completed form to your Departmental/School Ethics Committee for ethics approval (see pages 9, 11 and 13 of the Code of Practice).

Ethics Generic Framework

Please use this form where you require approval from the UEC or DEC/SEC for a series of investigation and/or teaching exercises involving human participants. Page 13 of the Code of Practice provides more information in relation to generic framework approvals.

Please use the form below to make a request to the UEC to devolve its responsibility for ethical approval to the DEC/SEC for certain categories of investigations which would normally fall within the remit of the UEC but where the DEC/SEC considers that it has the appropriate expertise to consider such investigations. Page 14 of the Code of Practice provides more information in relation to such applications.

Wording for information sheets where potential harm/serious crimes may arise

The following form of words for information sheets and advice to investigators should be used where information relating to harm and/or serious crime may come out in interviews:

There might be some questions that make you feel uncomfortable. You absolutely do not need to answer any question like that. The interviewer may when asking a particular question ask you if you are comfortable with that one. You can be assured that the only times an interviewer would ever have to pass on information to people like police and social workers is where you say something that shows you or another person is in immediate danger of serious harm (or some very, very serious crime is going to happen).

NHS Research Passports

Please note that staff and students undertaking their research within the NHS and whose work has a direct bearing on the quality of patient care are required to obtain a Research Passport prior to an Honorary Research Contract or a Letter of Access being issued by the NHS. The NHS organisation hosting the research will confirm when a research passport is required.

University ethical review of NHS investigations

Investigations within the scope of the Code of Practice and involving the NHS that do not require NHS approval under NHS procedures, must comply with the Code of Practice and should receive University ethical review pursuant to the Code of Practice.

Investigations outwith the scope of the Code of Practice and involving the NHS that do not require NHS approval under NHS procedures, may be ethically reviewed under the Code of Practice if the Chief Investigator wishes it.
