The Care and Support System for Patients and Carers (CARE-PAC) project can be visualised as a tree, rooted in the experiences of patients and carers and expertise of healthcare teams, adapting to changing environments of decline, transition and loss. Patients and carers are supported through the trunk of reliable, continuous monitoring, providing seamless communication and personalised support. The tree bears the fruits of positive outcomes—enhanced symptom management, compassionate, responsive care, reduced crises and increased sense of security for all.
